Humanitarian Use Device Worksheets

Submit these worksheets if using a Humantiarian Use Device (HUD)/Humanitarian Device Exemption (HDE).

Vendor Services Agreement

Submit two signed copies of the agreement if the project meets the Vendor Services Agreement Criteria.

Purchased Services Agreement

Submit two signed copies of the agreement if the project meets the Purchased Services Agreement Criteria.

Humanitarian Use Device (HUD) Agreement

Submit two signed copies of the agreement if using a Humanitarian Use Device (HUD).

Project Change Form

Submit this form if there is a change to the project that meets any of the criteria listed here.

Research Registration Form

Submit this form to SPA when a research patient is scheduled at an Allina facility. Contact SPA if your site has not completed research registration training.
