Initial Sponsored Project Review Worksheets

Submit one of the following applications/worksheets to request SPA review of a new sponsored project.

Form	Instructions
Vendor Project Setup Worksheets	Submit these worksheets if the project meets the SPA Vendor Project criteria. Instructions for completing the worksheets can be found in Part III of the SPA Guide.
Sponsored Project Setup Worksheets	Submit these worksheets if the project does not meet the SPA Vendor Project criteria and does not involve the use of a research medical device. Instructions for completing the worksheets can be found in Part IV of the SPA Guide.
Sponsored Project with Medical Device Setup Worksheets	Submit these worksheets if the project involves the use of a research medical device. Instructions for completing the worksheets can be found in Part V of the SPA Guide.
Humanitarian Use Device Worksheets	Submit these worksheets if using a Humantiarian Use Device (HUD)/Humanitarian Device Exemption (HDE).

Written Agreements

Form	Instructions
Vendor Services Agreement	Submit two signed copies of the agreement if the project meets the Vendor Services Agreement Criteria.
Purchased Services Agreement	Submit two signed copies of the agreement if the project meets the Purchased Services Agreement Criteria.
Humanitarian Use Device (HUD) Agreement	Submit two signed copies of the agreement if using a Humanitarian Use Device (HUD).

Project Change Form	Submit this form if there is a change to the project that meets any of the criteria listed here.

SPA Participant Reporting Forms

Research Registration Form	Submit this form to SPA when a research patient is scheduled at an Allina facility. Contact SPA if your site has not completed research registration training.
