Health care providers affiliated with Allina Hospitals & Clinics have the opportunity to be included in the Find a Provider feature of allina.com and the Allina Physician Referral telephone service. To be added to our provider referral directory, please complete the provider referral participation form and return it as instructed. Once submitted and approved, your information will be available to patients and other providers who call Allina Physician Referral at 1-866-904-9962 or visit www.allina.com/doctors.
To personalize your Web site profile, you may also include a photograph. Instructions for submitting your professional photo are included on the participation form.
