How do I request access to Physician Linkage via the Internet?
You may complete the on-line
request form. Please have your e-mail address ready when completing the form,
as this is a requirement for obtaining access.
Once you complete the form and submit it, an e-mail is sent to the Physician Linkage
team, who will process your request and return instructions to you in 5-7 business
days.
