The rehabilitation/psychiatry rotation is an elective rotation for pharmacy practice residents interested in the specialty of psychiatry and in rehabilitation therapy. The rotation is a minimum of 4 weeks, with an option for six weeks.
As the primary emphasis area of the rotation, residents provide pharmaceutical care for hospital patients in psychiatry, including three adult units and one child/adolescent unit. Secondarily, they also provide care for rehabilitation patients.
Goals
The rehabilitation/psychiatry rotation aims to develop a pharmacist who can:
1. develop and monitor drug therapy for safe and achievable outcomes in psychiatric patients
2. develop and monitor drug therapy for safe and achievable outcomes in rehabilitation patients
3. identify, analyze and resolve drug therapy problems in the psychiatric and rehabilitation populations
4. apply pharmacokinetic and therapeutic principles in nutritional support, antimicrobials, and anticoagulation
Objectives
increase his/her knowledge of psychopharmacology
increase his/her knowledge regarding the safe, therapeutic use of psychoactive drugs
identify the preferred treatment for adverse effects of psychoactive drugs such as neuroleptic malignant syndrome, serotonin syndrome and extrapyramidal side effects
demonstrate an ability to collect relevant information from other members of the health care team and laboratory in order to make appropriate drug therapy recommendations for the patients
increase his/her knowledge about various psychiatric illnesses and long-term social and physical sequelae of these diseases
Learning activities and responsibilities
While on rotation the resident will:
follow a decentralized pharmacist in the inpatient psychiatry stations and the inpatient rehabilitation station
attend nursing rounds for the inpatient psychiatry stations at least three times a week
provide drug information to health care professionals
attend care conferences as requested by the health care team
understand anticoagulation strategies in the rehabilitation patient population
provide an in-service to nursing staff on a topic of interest or a selected by the preceptor(s)
understand the uses of various psychiatric examinations
understand monitoring parameters and registration procedures with clozapine and the reporting systems in place for adverse effects
be able to contact pregnancy registries for psychoactive drugs, and the use of such medications in pregnant women
present and discuss pharmaceutical care plans on selected patients with preceptor(s)
participate in discussions with preceptors and read pre-assigned articles. Topics may include: psychiatric emergencies, mental health disorders, drug interactions with psychoactive drugs, cardiac issues in psychiatry, drug therapy in spinal cord injury patients, management of muscle spasticity and management of bowel/bladder in paralyzed patients
demonstrate knowledge of pharmacy references, have the ability to evaluate sources, and be familiar with literature searches
understand the relationship between the pharmacist and the pharmaceutical industry
potentially round with an individual psychiatrist and be available for questions and recommendations
