• If you think your privacy rights have been violated, or if you disagree with a decision about any of your rights, you may file a complaint. Please know you will not be penalized for filing a complaint.
• Please call 612-262-4900. You may also submit your compliant in writing and mail it to: Allina Hospitals & Clinics, P.O. Box 43, Minneapolis, MN 55440.
• You also may send a written complaint to the U.S. Department of Health and Human Services – Office of Civil Rights. For more information, go to www.hhs.gov/ocr/privacy/hipaa/complaints.
