Hospice is for people with a life-limiting illness and whose life expectancy is likely to be six months or less if the illness runs its natural course. Eligibility for hospice care is certified by your doctor and the hospice medical director.

Hospice care allows a natural death; it neither hastens death nor prolongs life, but rather provides comfort for those nearing the end of life.
Individualized plans of care, which honor patient’s goals, values and wishes, are created by the patient and family, the primary care physician and the interdisciplinary hospice team.
