Guidelines for hospice admission

  • The diagnosis of a life limiting illness. Medicare requires a prognosis of six months or less if the disease runs its natural course.
  • Patient desires hospice care.
  • Patient lives in the 28 county service area.
  • Patient has a primary contact person who will be responsible for decision making if the patients is no longer able.
