When you or a loved one face the challenge of serious illness, it is time to
find out about hospice so that informed decisions can be made. Having information
about hospice will help you and your loved ones understand
the ways that Allina's hospice care can help. Many families say
they wish they had started hospice services earlier.
Who is eligible for hospice?
Hospice care can be provided to anyone living in the Allina hospice 29 county service area. Patients are served in whatever location they currently call home, including assisted living centers, skilled nursing facilities, group homes and residential hospice facilities. Care is available to adults with all diagnoses, including: Alzheimer's disease, cancer, decline in clinical status, heart disease, HIV-AIDS, liver disease, neurological disease (ALS, MS, Huntington's, Parkinson's, etc.), pulmonary disease, renal disease and stroke/coma.
How is hospice care covered under Medicare?
Hospice care is covered under Medicare Part A (Hospital Insurance). You are
eligible for Medicare hospice benefits when you meet the following conditions:
You are eligible for Medicare Part A (Hospital Insurance), and
Your doctor and the hospice medical director certify that you are terminally
ill and probably have less than six months to live, and
You sign a statement choosing hospice care instead of routine Medicare covered
benefits for your terminal illness, (Medicare will still pay for covered benefits
for any health problems that aren't related to your terminal illness-see plan
specifics) and
You receive care from a Medicare-approved hospice program, like Allina Hospice.
