What to expect: Your first visit

The Allina Home & Community Services nurse will spend an hour or two with the patient and caregivers in their home setting explaining services and includes: 

  • a brief review of the patient’s present illness and a brief physical exam 
  • an explanation of how hospice care works, what services are covered, and how to get started 
  • if the patient and family want hospice care, several forms are then signed
  • the nurse assesses the need for equipment and medications; obtains the orders from the doctor, and places these orders
  • the nurse will leave information at the home including hospice team member names and telephone numbers, patient care instructions, a medication list and instructions, and a 24-hour nurse telephone line 
  • the specific team members will then contact the patient and family to set up the visit times.
