The Allina Home & Community Services nurse will spend an hour or two with
the patient and caregivers in their home setting explaining services and includes:
a brief review of the patient’s present illness and a brief physical exam
an explanation of how hospice care works, what services are covered, and
how to get started
if the patient and family want hospice care, several forms are then signed
the nurse assesses the need for equipment and medications; obtains the orders
from the doctor, and places these orders
the nurse will leave information at the home including hospice team member
names and telephone numbers, patient care instructions, a medication list
and instructions, and a 24-hour nurse telephone line
the specific team members will then contact the patient and family to set
up the visit times.
