Patients and families entering hospice and palliative care get the attention needed at this critical time.
A dedicated Residential Care Team
Exclusively serving patients in nursing homes and other long-term care settings, this team understands the special needs of patients, families and staff in this type of care setting. The communication and coordination of care between the staffs results in optimal care. Allina Hospice has contracts with over 100 facilities across the 28 county service area.
24-Hour triage assistance
This phone line is staffed with experienced hospice and palliative care RNs. They provide telephone assistance for patients, caregivers and referral sources and can make home visits at any time. This includes the expedition of order changes as well.

Culturally sensitive
Culturally sensitive end of life care is provided for those of diverse backgrounds. Special concern and interpretive services are available for non-English speaking persons.
A team of support
This care team includes the patient’s physician, the Allina Hospice medical director, registered nurses, pharmacists, home health aides, social workers, spiritual care coordinators, volunteer coordinators, volunteers, massage & music therapists, and bereavement services.
Palliative chemotherapy and radiation
These types of therapies may be appropriate to meet the goals of the hospice and palliative care patients. Our medical director can make home visits and is glad to discuss patient appropriateness for hospice or palliative care.
