Stroke recovery: Hospital discharge planning

You, your family and hospital staff all work on planning for your discharge. This planning begins early during your hospital stay.

Discharge planning is important. You and your family will decide on recovery goals and hospital staff can give you a list of resources to help aid your recovery.

Possible discharge options include:

  • home without the need for further assistance
  • home with visits from home care staff
  • home and outpatient therapy
  • assisted living apartment our building
  • rehabilitation in a nursing home/other health care facility
  • rehabilitation center
  • nursing home
