POLST: Physician Orders for Life Sustaining Treatment (free video conference)
Physician orders for life sustaining treatment (POLST) is a research-based, one-page document that translates a patient's health care wishes into a physician order that can be honored by community care team members.
Allina Hospitals & Clinics invites physicians, nurses, chaplains, social workers, case managers, discharge planners and other interested health care providers to learn Allina's goals for POLST. During this noon to 1 p.m. video conference, Gloria Cade, RN, director, Allina Hospice, will explain standard documentation of health care wishes for those who are frail, elderly, living in residential care centers or who have chronic advanced or end stage illness.
