Living with Diabetes  
You are a key player on your health care team when it comes to managing your diabetes.
 

Diabetes Education Program

After experiencing it herself, Sue Wolff recommends Allina Medical Clinic's Diabetes Education Program. Also endorsed by the American Diabetes Association®, the program can help you better understand and manage your diabetes.

These services are part of the care people with diabetes receive at Allina Medical Clinic.

  1. During a one-hour appointment at your clinic with a diabetes educator, you'll discuss your personal diabetes education and management needs.
  2. The diabetes educator will recommend small group or individual sessions based on your needs, treatment plan and type of diabetes. The sessions will be spread over time to give you a chance to practice new skills. If you'd like, you can invite a spouse, partner, family member or friend to come with you.
  3. Follow-up clinic visits will help you prevent diabetes complications. At least once a year, you should review your self-management plan with your diabetes educator and update it as needed. Every three months, you should see your doctor or health care provider to monitor treatment needs and answer questions.
